VITL Healthcare ("VITL," "we," "us," or "our") provides online platforms through which you may connect with independent, licensed healthcare professionals ("Providers") and access pharmacy services for medications prescribed to you (collectively, the "Services"). These Services constitute a form of telehealth, involving the delivery of healthcare services using electronic communications when the patient and Provider are not in the same location.

We believe telehealth offers significant benefits, including convenient and discreet access to licensed medical care. Telehealth may be used for diagnosis, treatment, follow-up, and/or patient education. This may include, but is not limited to:

  • Secure transmission of medical records, images, personally identifiable information, and protected health information.
  • Real-time or asynchronous interactions with Providers via audio, video, secure messaging, or email.

Potential Limitations of Telehealth

  • Certain medications or treatments may not be available due to state or federal regulations.

Patient Acknowledgements and Consent

By using our Services, you acknowledge and agree that:

  • You have read and understand this disclaimer, including the risks and limitations of telehealth.
  • You consent to receive medical care through telehealth from Providers affiliated with VITL Healthcare.
  • No specific outcomes or results are guaranteed; your condition may not improve and, in some cases, may worsen.
  • You are responsible for providing accurate and complete health information, including updates to your medical history and conditions. Providing false or incomplete information may result in harm, including serious injury or death.
  • Providers may determine that your condition requires in-person care or referral to alternative services.
  • Our Services are not a substitute for primary care or emergency services. If you are experiencing a medical emergency, you must call 911 or seek immediate care.
  • Your personal and health information will be collected, used, and shared as described in our Privacy Policy and permitted by law, including for treatment, payment, and healthcare operations.
